Something I missed originally - turn off "Subsurf UVs" when you manually
add a subsurf modifier to a sculptie. Also the scripts use the render
level on the subsurf modifier, it doesn't matter here as both are set to
level 2, but it's something else to watch for when doing stuff manually.

Ignore all the rubbish I spouted about the maximum bake range. Maya
isn't using the full range, but it wouldn't cause or fix the issues you
are seeing - I wasn't thinking clearly through the pain :(
